In order to properly close your current Hartford Insurance Company policy, attached please find a Cancellation Request. On the Cancellation Request, where areas HIGHLIGHTED, please fill in:
1. Your school name and mailing address.
2. Your policy No.
(You may find it on the invoice you have received or your previous Certificate of Insurance issued for School District's use.)
3. Sign and date at where it indicated.
4. When finish, please e-mail it to Tak Liong: liongt@gmail.com or fax it to 408-252-9885, we will process it for you.
5. If policy numbers cannot be located or any questions please call Tak Liong at 408-849-9421.
If you do not have your Hartford policy properly closed, Hartford will continue to issue you invoice plus late charges. Further ignorance will result in collection agency. Since ABC send most member schools their renewal invoices. ANCCS has no way to know which has renewed and which has not. We have then inform all non-profit schools to handle the cancallation by themselves. Sorry for all the inconvenience it has caused you. Your prompt attention to this matter is greatly appreciated.
